You’ve heard the brand name in the news: Ozempic® (semaglutide) is an FDA-approved GLP-1 medication indicated for managing type-2 diabetes and proven to support weight loss. If you’re prescribed Ozempic®, understanding how to properly administer the injection is key for ensuring safety and effectiveness.
The information in this article also applies for pre-filled pens of Wegovy® (semaglutide), Mounjaro® (tirzepatide), Zepbound® (tirzepatide), and certain other GLP-1 medications. These are just a few of the medications prescribed—where clinically appropriate and insurance eligible—by Calibrate clinicians for Calibrate members.
In this guide, we’ll cover everything you need to know about how to inject Ozempic®, including injection sites, how to inject Ozempic® in your stomach, thigh, or upper arm, and important safety information.

How to inject Ozempic® (semaglutide): The basics
Ozempic® is a once-weekly injectable medication that comes in a prefilled pen. The medication is injected under the skin (subcutaneous injection), and typical injection sites include the stomach (abdomen), thigh, or upper arm.
Once prescribed Ozempic® or a similar GLP-1 medication by your Calibrate clinical team, you’ll have a chance to ask questions or voice any concerns you might have about self-injecting with your team. But know that—while it can be intimidating at first—Ozempic® and other pre-filled GLP-1 pens are designed to be easy to use.
Most patients catch on within a matter of weeks and experience very little discomfort during the injection. That’s because the pens use ultra-fine needles, and the shot does not go as deep as, say, a flu shot: It only enters the fat layer, not the muscle beneath.
Injecting Ozempic®: Step-by-step guide
Here’s a breakdown of the steps for administering Ozempic® correctly, adapted from the manufacturer, Novo Nordisk:
- Choose an injection site: The common areas are the stomach (about 2 inches away from the belly button), the front of your thigh, or the outer part of your upper arm.
- Clean the injection site: Use an alcohol swab to clean the chosen area to reduce the risk of infection. Allow the skin to dry before proceeding.
- Prepare the Ozempic® pen: Remove the pen cap and check that the dose counter is set to zero.
- Attach a new needle: Screw a new needle onto the pen. Remove both the outer and inner needle caps.
- Set your dose: Turn the dose selector to your prescribed dose (the full dose is typically 0.5 mg or 1 mg per week, but follow your clinician’s instructions).
- Inject Ozempic®: Pinch the skin at the injection site, insert the needle at a 90-degree angle, and press the dose button to deliver the medication. Hold the button down for at least 6 seconds.
- Remove the needle: Once you've injected the full dose, release the button, and remove the needle from your skin. Dispose of the needle in a sharps container.
- Store the Ozempic® pen: Store your pen in a cool, dry place, away from direct sunlight.

Where to inject Ozempic® (semaglutide): Injection site options
The most common injection site options for GLP-1 medications are the stomach, the thigh, and the upper arm. If you need other alternatives for any reason, talk to your Calibrate clinician or the health professional who prescribes your medication.
Are there differences in efficacy of Ozempic® based on where I inject it?
You can expect similar results no matter where you inject Ozempic®. While some research shows slightly lower absorption when injected in the thigh compared to the stomach, this difference isn't considered significant.
Overall, injection sites—thigh, stomach, or upper arm—are interchangeable. Choose the site that feels most comfortable for you.
Other factors, like body weight and whether or not you have type-2 diabetes, may also affect how your body absorbs Ozempic®—but more research is needed to fully understand these differences.
1. How to inject Ozempic® in your stomach
The stomach is one of the most common injection sites for Ozempic®. To inject Ozempic® in your stomach:
- Choose an area at least 2 inches away from the belly button.
- Follow the general instructions for injecting, as described above.
Injecting in your stomach tends to be convenient for many users because the skin is easily accessible and you can keep an eye on the injection process.
2. How to inject Ozempic® in your thigh
Injecting Ozempic® in your thigh can be an alternative if you prefer to rotate your injection sites. To inject in the thigh:
- Select the front of your thigh, about midway between your knee and hip.
- Pinch the skin slightly to ensure the needle penetrates the subcutaneous tissue, not muscle.
3. How to inject Ozempic® in your upper arm
If you're comfortable or need assistance, the outer upper arm is another injection site. However, this area can be more difficult to reach by yourself, so you may require help from someone else to inject properly.
Important safety information and side effects
Potential side effects of Ozempic®
Like all medications, Ozempic® (semaglutide) may cause side effects. Common side effects include:
- Nausea and vomiting: These tend to be more pronounced when you first start the medication or increase your dose. Your body may adjust over time.
- Injection site reactions: Redness, swelling, or discomfort at the injection site can happen, though these are usually mild.
- Blood sugar changes: Since Ozempic® helps lower blood sugar, it’s essential to monitor your levels, especially if you're also taking insulin or other diabetes medications.
Rare but serious side effects may include the risk of thyroid cancer and pancreatitis. If you experience symptoms like severe stomach pain, swelling in your neck, or difficulty swallowing, seek medical attention right away.
Tips for injecting Ozempic® (semaglutide) effectively
- Rotate injection sites: Don’t inject in the same spot every week. Rotate between the stomach, thigh, and upper arm to reduce the risk of skin irritation or lipodystrophy (lumps under the skin).
- Hold down the dose button: Be sure to hold the button down for at least 6 seconds to ensure the full dose is delivered.
- Dispose of needles properly: Always use a sharps container to dispose of used needles to prevent injury or contamination.
- Keep your pen at the right temperature: Ozempic® should be stored in the refrigerator before its first use. After that, it can be kept at room temperature for up to 56 days, but avoid exposing it to heat or freezing temperatures.
