Calibrate is available to actively employed Brookfield employees and their dependents for whom the following apply:
General Eligibility (Applies to All Program Types)
To participate in any Calibrate program:
- Must be at least 18 years old.
- Must have medical coverage through Brookfield (Aetna PPO, Aetna EPO, or Aetna HDHP).
- Must not:Be pregnant, breastfeeding, or planning to become pregnant in the next two years.Have an active substance abuse problem.Have an active eating disorder (e.g., anorexia, bulimia, binge eating).Be undergoing cancer treatment.
- Must reside in the United States.
Eligibility for GLP-1 Medication + Calibrate Program Access
To qualify for the GLP-1 + Calibrate Program:
- BMI Requirement:
- BMI greater than 30 ORBMI greater than 27 with a co-morbidity, such as:Diabetes or pre-diabetesHigh cholesterolHigh blood pressureCardiovascular diseaseNon-alcoholic fatty liver diseaseObstructive sleep apneaPCOS
- Exclusions (Additional Restrictions for GLP-1 Eligibility):
- Cannot have had bariatric surgery in the last 18 months or complications due to bariatric surgery.Cannot have a history of pancreatitis in the last six months.Cannot have medullary thyroid cancer or MEN syndrome.
Eligibility for Calibrate Program Access Only (No GLP-1 Prescription)
To qualify for the Calibrate Program Only:
- BMI Requirement:
- BMI between 25–26.9 ORBMI between 27–30 without any co-morbidities.
- Exclusions (Same as General Exclusions).
If you have an Aetna EPO, Aetna PPO or an Aetna HDHP plan through Brookfield, you won't need to pay for Calibrate's program (valued at over $3000+) or for the cost of your doctor-prescribed medication—even if you have a deductible. Standard copays may apply for laboratory services.
No, your employer has taken extra steps to ensure your deductible doesn’t have to be met for your medication to be covered. If you have an Aetna PPO, an Aetna EPO, or an Aetna HDHP plan through your employer, your medication will be fully covered.
GLP-1 medication is a key component of Calibrate’s Two-Year Metabolic Reset. Our clincians prescribe a variety of GLP-1s, including semaglutide, tirzepatide, liraglutide, and dulaglutide. These GLP-1s are often referred to by their brand name, so you may have heard of these FDA-approved medications as Wegovy™, Ozempic®, Zepbound® or Saxenda®, to name a few.
GLP-1s mimic the effect of GLP-1 hormones naturally produced in the gut. These hormones send signals that affect metabolic function by regulating appetite and digestion to feel full faster and for longer, increasing your energy expenditure and maximizing nutrient absorption. These medications also work to regulate the body’s set point––the weight our bodies fight to maintain––and influence blood sugar response and insulin.
Yes, your employer-sponsored insurance plans cover GLP-1 medication.
If you have an Aetna PPO, an Aetna EPO or an Aetna HDHP plan, your GLP-1 medication will be fully covered. Importantly, you do not have to meet your deductible—if you have one—in order for your GLP-1 medication to be covered.
The Calibrate program is designed to fit into your life, not the other way around. You won’t be asked to make drastic changes. Instead, you’ll have the support of the Calibrate team to you make small changes to your habits that lead to big results.
Here’s a summary of what Calibrate requires of members in their program:
- Take medication as advised by the Calibrate Clinical Team, and respond to Clinical Team Check-ins via your Calibrate app every few months (if you're clinically eligible for GLP-1 medication).
- Attend 15-minute biweekly video coaching sessions
- Use the Calibrate app to track daily food, weight, and energy levels
- Read quick and simple lessons, released every two weeks
